Order of issue of the reference on temporary disability

The letter of Ministry of Health of Ukraine from 20.07.06 of No. 307 gave an explanation that due to the lack of forms of sheets of disability it is offered to provide the certificate of temporary disability of any form where requisites of filling of the reference are accurately defined.

Considering the situation which has developed in regions, in case of absence of forms of leaves of a netrudosposobyonost we suggest to provide the certificate of vreyomenny disability any for­my with the instruction:

• case of temporary disability "pervichyony" or "secondary";

• names and sites of an institution of a zdravookhyoraneniye;

• dates of issue of the reference on a temporary netrudosposobyonost (the figure specifies number then buk­vami – the name of month, is specified by figure year);

•  surnames, name, patronymic invalid (polyonost);

•  numbers of full years (figures);

•  names and sites of the enterprise, uchrezhyodeniye, organization (completely);

• diagnosis of the patient;

•  disability reasons;

• in case of hospitalization – gospitaliyozatsiya and extract dates from stationary office;

• at the direction on MSEK – direction dates signed by the chairman of VKK;

• dates of survey and the conclusion of MSEK signed by the chairman of MSEK certified by the press of MSEK;

• term of temporary disability; dates vyda­chi a disability leaf (number, month, year in the Arab figures), dates of extension of LN (number and month letters) and dates of closing of LN, podtverzhdenyony the signature and the press of the doctor (in the cases defined Inyostruktsiyey ~ the manager of office);

The order of delivery and registration of the reference on temporary disability should correspond to Instruction trebovayoniya about an order of issue of documents, udosyotoveryayushchy temporary disability of citizens.
